​Initial and yearly Physical in office is required.  

The practice will provide ongoing virtual care via patient portal messaging system for appointment requests, minor illnesses, minor skin conditions, follow up questions, and follow up studies with mild abnormalities to save time and avoid extra visits.

Patients initiated email, text message, phone, or video conferencing are not included.  If this is desired, please consider Direct Primary Care or pay per encounter.

The practice will continue to collect copay, deductible, coinsurance and bill insurances for all services rendered in person such as physical, chronic disease management, more in-depth visits, office procedures, ancillary services, and vaccinations.

Each child from 7-25 years of age with a parent enrolled in the package can be added for an additional $10 per month with one year commitment.​

Direct Primary Care

- $1200 per year​

  - Initial & yearly Integrative Physical is required in office   

  - Comprehensive wellness labs are included once yearly.

Comprehensive concierge medicine to pair with high deductible medical plans plus health saving accounts or for non-insured patients^

The practice will provide ongoing virtual care via patient portal messaging system, email, text message, phone, and video conferencing to address and coordinate all your concerns.

You can request appointments for phone conversation, video conferencing, or old-fashion in office visits for all your issues without additional charges during the normal business hours.   

Office procedures, ancillary services and labs are not included but the members get 70% discount on office procedures and ancillary services and 50% discount on labs*.

Each child from 7-25 years of age with a parent enrolled in the package can be added for an additional $35 per month with one year commitment. 

^This is an alternative way to receive your primary health care, but not a substitute for medical insurance.  We encourage, but not require, members to carry some type of health insurance policy in case of unexpected, expensive health events such as surgery or hospitalization.​

*The insured patients may elect to use their insurances instead.​
